STEP 1. ) Provide the following basic requirements

STEP 2.) Choose one of the 3 choices employed, own business, or OFW. If married, husband or wife must also choose and submit requirements.

- If husband and wife are working abroad different location both husband and wife must submit 1 consularized SPA form EACH.

NOTE: IF MARRIED husband or wife IS ALSO REQUIRED TO submit the SAME requirements. Choose if employed , has own business, or OFW.

Minor Requirements;
- 3 pcs. 1x1 pic. With signature at the back
- Latest 1 month pay slip original
- Certificate of employment and compensation
- Marriage contract 2 pcs.(photocopy)
- ITR(2008.2009)
